STEP 2. Fill Out Project Proposal Application Form (Form 1)
The Project Proposal Application Form (called Form 1) is the standard form required for an assessment. Form 1 requires information about the proponent and the project, including all associated activities, the project location, land ownership, existing environmental and socio-economic conditions, potential effects of the project, and any proposed mitigative measures. Form 1 can be filled out in two ways:
Option #1 - Form 1 on the YOR

- Go to the YOR by clicking here
- Read the introductions and familiarize yourself with the YOR process.
- Create a “new user” profile
- Follow the instructions
Option #2 - Form 1 printed version
If you are uncomfortable using the YOR at this time you can also fill out and submit Form 1 using one of the three methods below.
- Print it off, fill it out, and send it to a Designated Office via fax or regular mail.
- Fill it out on your computer and e-mail it to a Designated Office
- Visit a Designated Office and fill it out with the assistance of a YESAB staff person
STEP 3. Submit Form 1.
Once you have filled out Form 1 you must submit it to the YOR or send it to the local Designated Office.
